Dear Bergen County Foster Parents,

Under the reform plan DYFS is expected, among other things, to exercise the following case management tools:

> Build a Family Team .... involve EVERYONE important in the child's life in the case plan and meetings (including foster parents)

> Hold Family Team Meetings regularly and/or when requested by a family team member (a foster parent can request these)

> Advise children 14+ of educational, training, housing, and supports available

> Encourage children turning 18 to remain in care in order to receive education, training, and other supports available to them until 21

> Design case plan based on the child's health, safety, permanency, and well-being

> Visit the child on a monthly basis
